Obituary: Dr. Keith R. Hooker 2, 1939 – 4, 2012 Dr Keith R Hooker passed away peacefully at home 4, 2012 due to complications from his third bout with cancer. He enjoyed his last days surrounded by his family and loving wife, Nita. He wrote his own obituary which he wanted his family to use. So, here it is: If you are reading this, I have finally died. I was born at an early age and an only child to Ronald C. Hooker and Adela Joesting on 2, 1939 in Hull, Iowa. I grew up in Primghar, Iowa (population 800) until age 12 when my parents moved to Phoenix, Arizona. I graduated from Arizona State University, then medical school in Portland Oregon. I joined the LDS church while a medical student and, since then, the gospel and my family have been the center of my life. Ive been honored to have been entrusted with many church callings, the best one being scout master. a rotating internship in 1965 at Riverside Hospital in California, the years 1966-1970 saw me in Anchorage and Kotzebue, Alaska, treating tuberculosis and practicing general medicine for the Alaskan natives. Alaska, Phyllis and my four young children moved to Provo for a year or so at BYUs health center before joining the original group covering the Utah Valley Regional Medical (then known as LDS Hospital). For the next 40 years I felt lucky to get to work there, and thrived on the variety and challenges it presented. I always considered the administration and staff as part of my extended family and can imagine a better life. This led to being involved in the early days of EMS and teaching EMTs and paramedics. With the late Dr. Gene Davies, we started a rural health system with nurse practitioners living in each town and ER docs flying weekly to what eventually grew into eleven clinics. Early on I got involved in outdoor survival teaching at BYU and this led to co-founding Aspen Achievement Academy, the first of many wilderness programs Ive worked with. I married Phyllis Gunderson in medical school (1962) and we began our family. Phyllis Gunderson is the mother of my 8 kids and the worlds best grandmother to 36 grandkids. Each of my children has made me proud of them in their own way. Each of you is my favorite! I married Juanita (Nita) Miller in 2004, through eternity. Nita has been the joy of my life, and my loving companion for these last 8 years of my life. Weve enjoyed many adventures together and I will be waiting for her to join me in more adventures in the next world. Ive enjoyed, skiing, hiking, flying (with occasional ad landings) camping, mountaineering, (Utahns on Everest 1992). Other accomplishments that Im proud of: 1989, 1991 Emergency Room Physician of the Year; 2010 Utah Physician of the Year; 2011 State of Utah Physician of the Year. Keith is survived by his wife, Nita, his children Kyle (Kristen) of Heber, Utah; Kit (Sandy) of Colorado; Kelly (Dan Gish) of Pleasant Grove, Utah; Kiana (Mike Berrey) of Corvallis, Oregon; Keena (Cade Schaerrer) of Pleasant Grove, Utah; Karma (Bryant Martin) Las Vegas, Nevada; Kassie (Jerry Vance) of Salem, Utah; and Kory (Amy) of The Next World. He is also survived by his step-children Eva, Logan, and Dakota Miller; Diana Trotter, (married 1994-2001), her children Brett and Val Sorensen, Salem, Utah; Steph and Mitch Larsen, Provo, Utah; Lizzy and Tim Gatten, Idaho. Services will be , 9, 2012 at 10:30 a.m. in the Alpine Tabernacle, 100 East Main Street, American Fork. Friends and family may call at a visitation evening from 5-9 p.m. at Walker Sanderson , 646 East 800 North, Orem, or prior to the service from 9-10:15 a.m. Interment will be at East Lawn Memorial Hills .
